Warning signs of a gout flare

Some people with gout, also known as gouty arthritis, report that an attack begins with a burning, itching, or tingling sensation in the joint an hour or two before relapse begins. The joint may be a little stiff or a little sore. The characteristic signs of gout begin soon after. If you have repeated attacks, you will learn the signals your body is about to start.

Sometimes people with gout have no early signs that it is about to start. They may wake up in the middle of the night with a very painful joint.

When an exacerbation begins, most people experience severe redness, swelling, and pain, usually in one joint. The most common site for gout is at the base of the big toe, but it can occur in other joints such as the elbow, knee, wrist, ankle, and instep.

Home care for an exacerbation of gout

If your doctor has diagnosed you with gout and prescribed medication for an exacerbation, take the medication as directed when you know you have it. In most cases, this is likely to happen as soon as the first signs begin to appear.

Your doctor may prescribe non-steroidal anti-inflammatory drugs (NSAIDs) such as celecoxib, indomethacin, meloxicam, or sulindac, or suggest taking over-the-counter NSAIDs such as naproxen or ibuprofen. Depending on your medical history, your doctor may prescribe steroids or other medications to reduce inflammation, such as colchicine (Colcrys).

Uninterrupted

Just because you have an exacerbation doesn’t mean these drugs aren’t working. In the first few months that you take them, you may have a seizure as your body adjusts to the drug. Your doctor will likely give you something to take if this happens.

If you’ve been using gout medications for a long time and have flare-ups for the first time in some time, call your doctor. They can tell you about changing your dose or medications.

Pain relief without drugs

Use cold. If the pain isn’t that severe, try cold packs or compresses on the joint to reduce inflammation and relieve pain. It’s always a good idea to tell your doctor that you have a flare-up. Occasionally, you may need to take additional action to make sure your treatment plan is working or if your symptoms don’t improve. Call your doctor if:

This is your first outburst. There are many other conditions, such as joint infections, which share some of the same symptoms as gout attacks.

Diuretics

Diuretics help reduce blood pressure by flushing water and salt from the body. At the same time, they block the excretion of uric acid by the kidneys, which can allow uric acid to build up to gout-causing levels.

While switching to another blood pressure medication can help, some people get the best results with a diuretic, says Dr. Saag.

Managing a gout flare

Gout flare-ups are unexpected and painful; here’s how to get a handle on them.

Few things in life are more painful than a gout flare, so if you’re awakened in the wee hours by a joint that is tender, swollen, red and radiating heat, you’ll want to act fast. Here’s what you can do when a gout flare starts to ease the pain of and reduce the risk of others.

Take any medications you have on hand

Ice

Applying an ice pack to the aching joint can relieve pain and inflammation. Wrap the compress (even a bag of crushed ice or frozen peas) in a tea towel and apply to the area for 20-30 minutes, stretching several times a day.

Call your doctor. Tell your doctor immediately what’s going on. He may prescribe a new drug or come to the office for a synovial fluid test (to confirm a diagnosis of gout) or a corticosteroid injection to begin relieving inflammation quickly.
