If you’ve ever gotten cystic acne or an underground blind pimple, you’re probs very familiar with the feeling that is wanting to remove your own skin and borrow some from a baby (was that motivational? Inspirational? Worrisome? Sry—I’m breaking out right now, can’t help). Because cystic zits suck. They do. There’s no making friends with a pimple that throbs when you sleep and aches when you smile, then peaces out three months later, only to leave you with a red or brown splotch that takes half a year to fade.
And unlike every other form of acne, like blackheads, whiteheads, and little, squeezable-even-though-you-shouldn’t bumps, cystic acne can’t be treated with the same creams and serums in your medicine cabinet. What causes cystic acne?
Ugh, bad luck? Just kidding. Maybe. “Fundamentally, everyone’s skin is the same—everyone has hormonal fluctuations, oil productions, and acne-causing bacteria in their skin that could cause a huge breakout at any time,” says Mona Gohara, MD, associate clinical professor at Yale School of Medicine. But, BUT! “The sebaceous glands in some people are just naturally more sensitive to these triggers, causing a larger and more frequent inflammatory response.”
And if you’re someone with cystic acne, here’s what that inflammatory response looks like: Your follicle gets pissed off and clogged, trapping the oil and bacteria deep within your pore and causing intense inflammation. That inflammation then ruptures the sides of the pore and leaks bacteria into the surrounding skin, resulting in a big-ass, super-painful cystic zit with no escape route. “Honestly, your risk of developing cystic acne only partially has to do with how you take care of your skin,” says Dr. Gohara. Translation: It’s not your fault.
How to get rid of cystic acne fast
K, first, make sure to temper your expectations. Though you’ve got a few at-home treatments that can help prevent cystic acne from forming (more on that later), there’s not much you can really do at home to get rid of the bump that just popped up. That being said, here are your best at-home cystic acne treatments that can help:
1. Don’t try to pop it. Seriously
Cystic zits literally cannot be popped, since the problem is so deep within your skin. “Trying to manipulate or squeeze the cyst will only push the inflammation out further, leading to more severe scarring and a possible infection,” says Shari Marchbein, MD, a dermatologist in New York City. Even if it has the tiniest of whiteheads on top, don’t trust it—that sh*t is deep, and you’ll never get it out. You will always (always) make it worse.
2. Ice it
Nah, it’s not a bunch of B.S.—ice really can help your cystic breakout…to some extent. “The reason a cystic pimple is painful is because of swelling and inflammation in the tissue,” says Dr. Gohara. “Ice can help constrict your blood vessels and bring down the swelling a bit.” Wrap an ice cube in a paper towel or napkin (no direct-to-skin contact) and hold it against your zit for 1-2 minutes max, giving your skin 30-minute breaks in between.
BUT WHAT ABOUT WARM COMPRESSES?! The thing mothers and Google have been telling us to put on our zits for decades?! Sry, but both derms gave me a “meh” response: “It’s not going to help heal your cystic zit faster, and I don’t use them on myself when I get cystic breakouts either,” says Dr. Marchbein. Dr. Gohara agrees, adding “heat can be really inflammatory, so I’d only apply a warm compress once or twice to relieve some pain if needed.”
3. Cocktail a spot treatment
K, don’t get too excited—no topical product will get rid of your cystic zit at home, aside from time (it’s free!). BUT, “a cocktail of benzoyl peroxide and 1 percent hydrocortisone cream can help flatten the zit faster,” says Dr. Gohara. Both are anti-inflammatories (yup, even benzoyl peroxide) that will help calm the bump, and “the less inflammation you have now, the less hyperpigmentation and scarring you’ll have when the zit heals,” she says. So once a day for one week, dab benzoyl peroxide on the cyst, let it dry, and then tap on a thin layer of hydrocortisone cream.

Common types of acne include whiteheads (closed comedones). It occurs when dirt or excess oil (sebum) is trapped below the skin surface, forming a firm white bump. The blackish color comes from oxidization when the air reacts with melanin, a pigment in sebum. - Avoid touching your face. Now I know this is hard especially when you are self-conscious, but let’s view it this way, touching will only worsen it. Dirt and bacteria on your hands can clog your pores and make acne worse if you constantly touch your face. If your skin feels irritation from acne, use a gentle oil-free facial wipe daily to remove excess dirt and calm your skin.
- Do not squeeze or pop your blemishes or you may risk developing scars. Squeezing a pimple might even spread the bacteria further.
- Choose the right cleanser. Use a mild non-soap cleanser (many soap cleansers are free from harsh chemicals, using natural ingredient are in your best interest. Harsh soaps and scrubbing can irritate your skin and make acne worse.
- Wash regularly. Wash your face morning and night whether you have make up on or not. With make up on, don’t just wash, wash thoroughly with soap after you must have cleansed with makeup remover or wipes.
- Use the right skin care product. Apply oil-free moisturizer if your skin is dry or itchy. An astringent is only recommended when you have oily skin and even for the astringent you need, you have to ask your skin expert which is good for you depending on your skin type. People with non-inflammatory acne, like whiteheads and blackheads that do not cause redness, can use mild exfoliating products that are available. People with dry, sensitive skin should limit exfoliating to once or twice per week, while people with oily, thicker skin can exfoliate once a day.

It’s common to have low levels of zinc, but taking multivitamins and eating a healthy diet should give you all the zinc you need. Best dietary sources of zinc are seafood, beans, cooked greens, pumpkin, mushrooms.
Drink green tea: Green tea contains many antioxidants that prevent aging and protect the skin. Though not directly linked to acne prevention, it nourishes the skin. You have got absolutely nothing to lose. Rather it will help your skin look fresher and younger.

You can also use a touch of witch hazel on a cotton ball or swab. Carefully dab the popped pimple with this solution a few times a day, at least until a good scab forms. Again, don’t aggressively rub at it or disrupt the forming scab.
Although lots of people suggest cleaning a popped pimple with rubbing alcohol or hydrogen peroxide, those can sting and aren’t as effective as witch hazel at reducing inflammation.
Apply Ice If Swollen
If you’ve really gone to town squeezing at a blemish, you probably have a big, swollen, red bump on your face right about now. Ice is the best way to soothe it down and reduce inflammation.
Use an ice cube or cold pack, either wrapped in a soft cloth or paper towel. Apply it over the swollen area for a few minutes at a time, several times a day. This will help take down the swelling and make your popped pimple look and feel a whole lot better.
If your popped pimple isn’t swollen and already has formed a scab, icing the area won’t do anything so you can just skip this step.
Dab on an Antibiotic Ointment
Treat your popped pimple-like an open wound, because that’s basically what it is. An over-the-counter antibiotic ointment is your best friend.
Dot a tiny amount directly on the popped pimple or scab. This will help speed up healing time. It also keeps the scab moist, so it won’t look as dry, crackly, and obvious. Keep a picked-at pimple covered with a small amount of ointment until it’s fully healed. This can take a few days.
Also, make sure you’re only covering the pimple, not the skin around it (especially if it’s on your face). Antibiotic ointment may clog your pores, so you want to keep it just in the area where it’s needed.
Leave the Scab Alone
It can be tempting but don’t mess with the scab. You need that scab to stay put for your pimple to heal. Each time the scab is removed, the healing process has to start all over again putting you back at square one.
If you’re a bonafide picker, do your best to keep your hands off your face. Again, keeping the scab covered with a healing ointment makes it less appealing to pick it off.
Continue Using Your Acne Treatments
If you’re using any over-the-counter (OTC) acne products or prescription acne medications, you should continue to do so. Most acne medications have antibacterial properties which should be helpful in getting that pimple to heal.
If you treat your popped pimple well, it should heal within a few days. Try to be patient until this happens.
